What is the Hospital Safety Score?

The Hospital Safety Score is an A, B, C, D, or F letter grade reflecting how safe hospitals are for patients. The Hospital Safety Score was launched for the first time in June 2012.  Eligible hospitals receive a Hospital Safety Score twice a year based on publicly available data.

Measures Included in the Score The Hospital Safety Score includes publicly available measures of patient safety. Measures are national in scope and reported at the hospital level. Primary sources include data from CMS Hospital Compare and the Leapfrog Hospital Survey, as well as secondary sources such as data from the American Hospital Association’s Annual Survey.

Data Snapshot Dates

The data snapshot date is the date that Leapfrog will extract public data (i.e. download data published by CMS or submitted to Leapfrog or secondary data sources) to calculate Hospital Safety Scores.  

February 15, 2016 (for April 2016 release)
August 31, 2016 (for October 2016 release)

Courtesy Hospital Preview Dates

As a courtesy to hospitals, Leapfrog will hold a 3-week data review period before the public release of each Hospital Safety Score update. The data review period is an opportunity for hospitals to review the data for accuracy (i.e., identify recording errors, hospital name and address changes, etc.) and review changes to the scoring algorithm. In addition, hospitals will be able to preview their letter grades 48 hours prior to public release. 

March 2-23, 2016 (for April public release)

2016 Leapfrog Hospital Survey opens on April 1, 2016 

Public release of Spring 2016 Hospital Safety Score letter grades using February 15, 2016 snapshot data

JUNE DEADLINE: 2016 Leapfrog Hospital Survey submission deadline is June 30, 2016**
JULY CMS scheduled to refresh data on Hospital Compare website
 
AUGUST

Snapshot Date: August 31, 2016 (for October 2016 release)

PLEASE NOTE: Hospitals that would like Leapfrog Hospital Survey data included in the Fall 2016 Hospital Safety Score MUST submit a 2016 Leapfrog Hospital Survey on or before August 31st in order for it to be used in the Score calculations. Surveys submitted or updated after August 31st will not be used in the Fall 2016 Hospital Safety Score calculations.
SEPTEMBER

Courtesy Data Review Period for October public release

OCTOBER Public release of updated letter grades using August 31, 2016 snapshot data
 
DECEMBER

CMS to update data on Hospital Compare website.  

DEADLINE: The 2016 Leapfrog Hospital Survey will close on December 31, 2016. No 2016 survey can be submitted after this date.

**June 30, 2016 is the deadline for the 2016 Leapfrog Hospital Survey. Hospitals that are targeted to submit a survey, and do not complete one by June 30, 2016, will be publicly reported on Leapfrog’s consumer website (www.leapfroggroup.org/compare-hospitals) as “Declined to Respond.” Hospitals can continue to submit new surveys and update previously submitted 2016 surveys until December 31, 2016.
